Form can be printed out or downloaded to fill out, then printed and mailed or emailed as an attachment. To scan the completed form, please save as a PDF file. Keep a copy for your records, and include one copy in your wine or spirits shipment. Entries made through Enofile or TheDataStill can be paid online or by mailing a check or sending credit card information to Sommelier Challenge by mail, phone or email. Payment is due at the time of entry. Trucks delivering shipments on pallets must have liftgates. Sender is responsible for all delivery costs. Please include a copy of your entry form in the shipment.
